Community Outreach & Emergency Referral
Not every family can get to care. So we bring care to them and when every minute counts, we make sure they can get to it fast.
Community Health & Outreach
Healthcare that stays inside a clinic doesn't reach the families who need it most.
Our health workers go where the need is — leading education sessions on maternal health, hygiene, nutrition, and disease prevention, and working alongside local leaders, schools, and community volunteers to promote healthy practices at the household level. We identify at-risk mothers and children early, screen for common illnesses and malnutrition, and connect families to facility-based care when they need more than outreach can offer. This work reaches deep into rural communities, displacement sites, and other hard-to-reach populations — and we invest directly in the community health workers and volunteers who make it possible.
What this includes:
Health education and awareness on disease prevention and healthy practices
Maternal, newborn, and child health outreach
Hygiene, nutrition, immunisation, and reproductive health campaigns
Community-based screening for illness and malnutrition
Early identification and referral for specialised care
WASH promotion for safe water, sanitation, and hygiene
Training and support for community health workers and volunteers
Ambulance & Referral Services
When every minute counts, getting there safely is everything.
Our 24/7 ambulance service ensures mothers, newborns, and other patients facing emergencies can reach the specialised care they need — stabilised, safely transported, and handed over with everything the receiving team needs to continue their care without delay. Whether a case comes through community outreach, a primary health facility, or a direct emergency, the same standard of coordination applies.
How a referral works:
Assessment — a patient's need for emergency or specialised care is identified by our health teams or community outreach workers
Referral decision — the right facility is chosen based on the level of care required
Ambulance dispatch — a fully equipped ambulance and trained personnel transport the patient safely
Coordination — the receiving facility is contacted ahead of arrival, so they're ready
Handover — the patient arrives with full documentation and medical information in hand
Follow-up — we track outcomes to close gaps and keep improving the service
Community feedback — patients and caregivers can share their experience, helping us do better
